Appointments

To make an appointment at LiveWell Pain Management Centre, please call (07) 4724 2592 during business hours or email the clinic at reception@livewellpmc.com.au and we will do our best to schedule an appointment time that will best suit you.

 

All new patients of LiveWell are required to fill out our New Patient Questionnaire Form. Usually we ask you to arrive 10mins prior to your appointment to complete this, however please feel free to download our form here and bring it in with you.

 

Also, here at LiveWell Pain Management we understand that sometimes you may not be able to keep your appointments. Please make yourself familiar with our Cancellation Policy to avoid any late cancellation or failure to attend fees.
